Pain across the heart is read through the heart as the organ of love, an old love-wound resurfacing. The pattern is read as buried grief around love pressing on the heart: a belief that one is not loved or worthy of love, secret or wounded loves that seem to "attack" the heart, and a clinging to the hurt rather than letting it flow. The short explanation's "excessive emotional and mental stress" is that accumulated, love-wounded heart. Relief is described as letting the old grief surface and release, letting love reach the heart beginning with oneself, and loosening the grip on the hurt.
