Fear of life.
An enlarged heart is read as a heart overstretched from carrying too much - the organ of love straining to hold more than it can. The pattern is read as a heart enlarged by trying to earn love and to carry everyone, beneath a belief that one is not loved enough or worthy of love, and a pervasive fear of life met as a constant battle. The short explanation's "fear of life" is that braced, overburdened, love-hungry heart. Relief is described as letting love reach the heart freely rather than working for it, meeting life as less of a battle, and no longer asking the heart to carry it all.
