Pain across the heart is read through the heart as the organ of love itself - the centre where one both gives love and feels loved. The ache is read as the heart's response to a specific wound around love: a quiet conviction that one is not loved, that one is not worthy of love, or that one's own love will not be answered. From that belief the heart is read as beginning to close - the way it is read as closing even very early in life, when a child first senses love withheld - while one tries instead to earn love through effort, taking life heavily and feeling one's heart "torn out" or starved when the love one works for does not return. The short explanation's "excessive emotional and mental stress" is that over-burdened, under-loved heart, straining to win what it believes it lacks. Relief is described as letting love reach the heart again beginning with oneself, meeting the belief that one is unworthy of love directly, and recognising that love is given and received freely rather than earned - so the heart no longer has to strain or close to protect itself.
